29 CFR 1917.95(c): The employer did not provide well maintained emergency bathing, eye washing or other facilities where employees were exposed to hazardous substances. Wash bay, Maintenance shop: On or about 19 December 2012 and times prior thereto, the emergency eyewash station in the bay froze and water supply lines were broken. This condition exposed delays in employees to utilize the eyewash.

29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested Chassis and Maintenance Shops: On or about 19 December 2012 and times prior thereto, yearly fit testing was not conducted for maintenance personnel required to wear respiators. This condition exposed employees to inhalation of welding fumes and other chemical hazards.
