29 CFR 1917.154: Compressed air for cleaning exceeded 30 psig:  Chassis Stall: On or about August 12, 2014 and at times prior thereto, compressed air tool used for cleaning was set at 120 p.s.i.g.  This condition exposed employees to air embolism hazards in the event of skin contact.

29 CFR 1917.112(b)(1): Guardrails were not provided at locations where employees were exposed to floor or wall openings or waterside edges, including bridges or gangway-like structures leading to pilings or vessel mooring or berthing installations, which presented a hazard of falling more than 4 feet or into the water:    (a)  Pier 3, Overhead Gantry Crane: On or about August 13, 2014 and at times prior thereto, guardrails were not provided at two (2) 10.5 in wide locations on top of the crane operators cab.  This condition exposed employees to fall hazards of 70 feet.    (b)  Pier 3, Overhead Gantry Crane, Washrack Platform: On or about August 13, 2014 and at times prior thereto, guardrails were not provided at the rear of the platform.  This condition exposed employees to a fall hazard of 62 feet.        Horizon Lines of Alaska, LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 962574, citation number 1, item number 1 and was affirmed as a final order on April 28, 2014, with respect to a workplace located at Anchorage, Alaska.    Horizon Lines of Alaska, LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 801542, citation number 1, item number 2 and was affirmed as a final order on February 8, 2013, with respect to a workplace located at Anchorage, Alaska.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:  Chassis Stall: On or about August 12, 2014 and at times prior thereto, employee that voluntarily used a Miller Fresh Air Hood powered-air purifying respirator was not provided a medical evaluation.  This condition exposed employees to limited information on physiological alterations respirators cause.    General Industry standard applicable to Marine Terminal employment.
