29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: On or about December 13 of 2012 at the 77 Ella Grasso Turnpike construction site in Windsor Locks, an employee was observed working from a Versalift bucket truck elevated at heights from 7 feet 6 inches to 18 feet and the personal fall arrest system in use was not rigged to prevent the employee from contacting lower levels.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used: On or about December 13 of 2012 at the 77 Ella Grasso construction site in Windsor Locks, employees were not trained, as part of their fall protection training, on performing the fall distance clearance calculations required when erecting a fall protection system.
