29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: MANUFACTURING AREA: No training was provided in regard to Appendix D of 1910.134 in the following circumstances: A) On or about December 20, 2012, dust masks were available to employees for voluntary use while the concrete floor was being cut and earth below was being excavated prior to installation of a thicker floor slab. B) On or about April 10, 2013, an employee was observed wearing a dust mask while working with parts in an oil bath. Employee stated she was using it voluntarily.

29 CFR 1910.134(d)(1)(i): Selection of appropriate respirators was not based on the respiratory hazard(s) to which the worker was exposed and user factors that affect respirator performance and reliability: MANUFACTURING AREA: An employee was observed by the CSHO wearing a paper dust mask very loosely over her nose and mouth reportedly to minimize the oil vapors she breathes in. Employee also had an N-95 particulate respirator at her work station.
