You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. Located at KDC Tritech in New Albany, OH: a. On the 2012 OSHA 300 log, the employer did not ensure that information was recoreded in column F that was required for each injury. 1. Case number 2, BP on 6/27/12, did not include the cause of the injury and which hip and thigh were injured. 2. Case number3, VB on 8/09/12, did not include the actual injury to the right foot. 3. Case number 14, GP on 8/22/12, did not include what caused the injury to the elbow. 4. Case number 21, AC on 10/20/12, did not include the actual injury to the eyes. 5.
