Located at the Capmatic RR Quattro Overcapper, serial number RR15122268, on Line 6 in the Production Department, the company did not have documented procedures for the control of hazardous energy for employees that performed service/maintenance activities on the Capmatic, thereby exposing the employees to an unexpected startup, release of stored energy, etc. while performing service/maintenance activities.
