29 CFR 1926.451(e)(1): For scaffold platforms more than two feet above or below a point of access, portable ladders, hook on ladders, attachable ladders, ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personal hoist, or similar surface were not used: (a)West side of the residential home: Employee was exposed to fall hazards of 13'1" while working on a tubular welded scaffold platform which was not provided with a ladder for safe access. Employee climbed the Mason Type Tubular Welded Scaffold Frames as a means of access.
