29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    On or about January 9, 2013 at the above address site:  An employee performing roof work on a low slope roof with unprotected edges approximately 14 ft.  above a lower level was not protected from falling to that lower level.

29 CFR 1926.1053(b)(8):  Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:    On or about January 9, 2013 at the above address site:  Employees gaining access to and from a low slope roof approximately 14 ft. above a lower level using an 18 ft. multi-use extension ladder set up on an active walkway did not have that ladder secured to prevent accidental displacement.

29 CFR 1926.1053(b)(22):  Employees  using ladders were carrying objects or loads that could cause the employee to lose balance and fall:    On or about January 9, 2013 at the above address site:  An employee climbing an 18 ft. aluminum multi-use extension ladder to gain access to a low slope roof approximately 14 ft. above a lower level was carrying a hammer drill that could cause him to lose balance and fall.
