29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed    On or about March 1, 2016, at the above addressed jobsite, employees performing roofing work were exposed to a fall hazard of about 17 feet while using an extension ladder that did not extend 3 feet above the landing.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder.    On or about February 19, 2016, at the above addressed jobsite, an employee who had been performing carpentry work, fell about 17 feet from a ladder that had not been properly secured.       Infinity Roofing and Sheet Metal, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(8), which was contained in OSHA inspection number 817602, citation number 1, item number 2, and was affirmed as a final order on 03/01/2013, with respect to a workplace located at 2540 SE Federal Highway, Stuart, Florida, 34994.
