OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to point of operation hazards associated with unprotected motor controls for the permanent mold casting machines: Location: The Bracket Dept. at or near 2100 Walton Rd., St. Louis, MO 63114. The employer did not ensure that the motor controls palm buttons of the six (6) tilt pour casting machines identified as PM 21, PM 22, PM 23, PM 32, PM 33, and PM 36 were protected from accidental activation. Among other methods to correct the hazard include, but are not limited to: 1.) Install shrouds/ring around the palm button controls 2.) Implement the requirements of ANSI B152.2-1982 for motor start controls.

On or about Aug. 23, 2012, a maintenance employee received injuries to the toes on the right foot while working on the hydraulic cylinder in the Sand Molding area. Injuries included a fractured great toe, amputation of the 2nd toe, and laceration of the 3rd toe.

On or about Aug. 23, 2012, a maintenance employee received injuries to the toes on the right foot while working on the hydraulic cylinder while entering the MH4/Shamrock machine work cell in the Sand Molding area to determine the cause of a jam while the equipment was in manual mode. Injuries included a fractured great toe, amputation of the 2nd toe, and laceration of the 3rd toe.
