29 CFR 1910.23(c)(1):     Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides,    a. At the Inca Presswood facility located in Dover, Ohio the employer had a catwalk that ran along side the boiler above the furnace that was not protected by guardrail along the inside of the catwalk where maintenance employees walked.  The catwalk was an approximate 6 foot fall to the top of the furnace.

At the Inca Presswood facility located in Dover, Ohio the employer had a catwalk that ran alongside the top of Press # 1 did not have guardrails where the ram for the top die travels up and down which was located in proximity to where maintenance employees walked.    b. 29 CFR 1910.212(a)(1):     Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    a. At the Inca Presswood facility located in Dover, Ohio the employer had employees exposed to a caught-in, crushing injury while working with the Pac-Head machine.  The Pac-Head machine created a nip point where it picked up and moved the product to the boxes.

Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.    a. At the Inca Presswood facility located in Dover, Ohio employees were exposed to an unguarded pulley directly adjacent to the catwalk that ran alongside Press # 5 while breaking jams or inspecting equipment.

29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.    a. At the Inca Presswood facility employees were exposed to an electrical hazard on the back side of the flaker machine in the Chip Preparation Room where there was a flexible cord going to an electrical disconnect that was pulled from box causing strain on the terminals.    b. At the Inca Presswood facility employees were exposed to an electrical hazard on the control panel for the Shredder in the yard where there was a flexible cord that was going into the bottom of the panel that was pulled from the box causing strain on the terminals.
