29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) Murphy Road Job Site - On or about 1-18-13 - Employees were observed installing metal sheet panels from a Terex aerial lift at a height of 10 to 20 feet above the ground using a harness and 6 foot shock absorbing lanyard. Employee would hit the ground or section of the building before being stopped by fall protection system in use.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used: a) Murphy Road Job Site - On or about 1-18-13 - Employee was installing metal sheet panels from an aerial lift, 10 to 20 feet above ground, with a fall protection system that would not protect him from contact with the ground. Six foot lanyard with shock absorbing system was not adequatae protection for use in aerial lifts.
