29 CFR 1926.453(b)(2)(vi): Boom and basket load limits specified by the manufacturer were exceeded: On or about 07/24/2019 at the Sullivan Honda Construction Project, employees were using an aerial lift to access and descend to and from the roof deck and the three employees overloaded the load limits of the aerial lift basket.

29 CFR 1926.760(d)(1): Guardrail systems, safety net systems, personal fall arrest systems, positioning device systems and their components did not conform to the criteria in �� 1926.502 (Appendix G to this subpart). On or about 07/24/2019 at the Sullivan Honda Construction Project, employees were using personal fall arrest system in an aerial lift and the personal fall arrest systems in use allowed contact with lower levels.
