CSI Industries Inc., 6910 W Ridge Rd, Fairview, PA 16415, spray paint booth, on or about January 18, 2013 and times prior thereto: Flammables and combustibles, including, but not limited to, three drums of lacquer thinner, were stored in the same room with spraying operations. The quantity of lacquer thinner in the room exceeded daily use amounts. B.) CSI Industries Inc., 6910 W Ridge Rd, Fairview, PA 16415, spray paint booth, on or about January 18, 2013 and times prior thereto: The quantity of paint products in the same area of spray operations exceeded daily use amounts. These paint products, included, but were not limited to, containers of Buff W/R Air Dry Primer, RAL 7040 Window Grey W/R Air Dry Paint, and Pantone 647C Blue W/R Air Dry Paint.

CSI Industries Inc., 6910 W Ridge Rd, Fairview, PA 16415, spray paint booth, on or about January 18, 2013 and times prior thereto: Electrical junction boxes with missing knockouts were located in the area where spray painting operations were conducted with flammable and combustible liquids. The electrical junction boxes were not approved for Class I hazardous locations. C.) CSI Industries Inc., 6910 W Ridge Rd, Fairview, PA 16415, spray paint booth, on or about January 18, 2013 and times prior thereto: Daisy chained and damaged flexible cords were located in the area where spray painting operations were conducted with flammable and combustible liquids.
