29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition:    JOB SITE: 615 Old St. Mary's Road, Perryville, MO 63775, in the shipping and receiving area of this facility, a safety check was conducted on a Hyster Electric Forklift, Unit #264, and revealed the forklift's emergency brakes were defective; with emergency brakes engaged, the forklift continued to move forward and in reverse when the unit was gear and accelerated.  The workers were exposed to a struck-by hazard.
