This condition exposed employees to inhalation hazards.

The employers written hazard communication program had not been reviewed and/or revised on an annual basis e.g. program last review and/or revise date was April 8, 1998.  As a result the maintenance shop used and maintained petroleum, oil and lubricants that were not included in the program.
