29 CFR 1910.110(b)(10)(ix): Safety relief valves shall have direct communication with the vapor space of the container at all times: a) Bulk Plant at Highway 32 West - On June 10, 2013, the employer did not ensure that the pressure relief valve on the ASME propane tank involved in the propane explosion was in constant communication with the vapor space of the tank at all times in that the tank was inverted upside down during transfer operations, exposing employees to fire and burn hazards. ABATEMENT CERTIFICATION AND DOCUMENTATION IS REQUIRED
