Parking Area in Front Office Trailer: An employee using the Hitachi circular saw to cut wood shims was plugged into the duplex receptacle in the office trailer and there was no ground fault circuit interrupter protection as determined on 1-29-13.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a.Corridor 200: Employees were using the cross bracing to gain access to the top level of the fabricated frame scaffold which was 9 feet 7.5 inches off the ground as observed on 1-29-13.
