29 CFR  1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift.  a) Back side of school building; On or before August 8, 2025,  an employee was operating an aerial lift to survey area and ensure the lift could reach the areas to be pressure washed. The employee was not wearing a safety harness or tied off to the basket of the lift, exposing the employee to falls up to approximately 15 feet.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
