On or about January 23, 2013, the employer did not ensure employees exposed to open-side floors approximately 5, 10 below ground level, located on the north and south side of the line #12 pre-notch press, were guarded by a standard railing or equivalent when placing lock out/tag out devices and performing maintenance on the pre-notch press.
