29 CFR 1926.451(g)(1):  Each employee on a scaffold 10 feet above a lower level was not protected from falling to the lower level:    (a)Soto Siding @ 154 Sycamore, Johnstown, CO 80534: On and before January 22nd, 2013, the exposing employer did not ensure that an employee was protected from a fall hazard in that guardrails or other suitable fall protection was not installed on the ends of a pump-jack scaffold on the south end of a single-family home under construction.  This condition exposed an employee to the hazard of a fall of 15 feet.

29 CFR 1926.454(a):  The employer did not have each employee who performs work while on a scaffold(s) trained by a person qualified in the subject matter to recognize those hazards associated with the type of scaffold(s) being used and to understand the procedure(s) to control or minimize those hazards:    (a)Soto Siding @ 154 Sycamore, Johnstown, CO 80534:  On and before January 22nd, 2013, the exposing employer did not ensure that an employee was provided with adequate training regarding pump-jack scafflding.  This condition exposed the employees to a fall of up to approximately 15 feet due to not recognizing the hazards of working on the scaffold and being exposed to a fall hazard from falling off the end of the platform.    Abatement Note:  The training shall include the following elements at a minimum:    (1)	The nature of any electrical hazards, fall hazards, and falling object hazards in the work area;    (2)	The correct procedures for dealing with electrical hazards and for erecting, maintaining, and disassembling the  fall protection systems and falling object protection systems being used:    (3)	The proper use of the scaffold, and the proper handling of materials on the scaffold;    (4)	The maximum intended load and the load-carrying capacities of the scaffold used; and    (5)	Any other pertinent requirements of this subpart.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection measure provided.     (a)Soto Siding @ 154 Sycamore, Johnstown, CO 80534: On and before January 22nd, 2013, the exposing employer did not ensure that fall protection systems were utilized by employees engaged in siding operations off of a pump-jack scaffolding. Employees were required to gain access through a window to the pump-jack scaffolding erected on the south end of a single-family home under construction. The employee was exposed to a fall hazard of approximately while walking from the window to the pump-jack scaffolding. The condition exposed employees to a fall hazard of approximately 10 feet.    Soto Siding was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 392262, citation number 1, item number 1b and was affirmed as a final order on June 16th, 2012, with respect to a workplace located at 601 Stoney Brook Rd., Fort Collins, CO 80525.
