29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Basement Stuffing Department- The employer failed to ensure that all areas where hazards were created by the continuous spinning of the setting cylinder on machines, such as on the Rizzi Setting Machine where the employee contacted the spinning setting cylinder, were provided with guarding to protect the operator from injury.      b) Basement Stuffing Department- The employer failed to ensure that all areas where hazards were created by pinch points, ingoing nip points and rotating parts on machines, such as on the Woburn #1 Setting Machine and the Woburn #2 Setting Machine where covers over hazardous areas were not secured or affixed, were provided with guarding to protect the operator from injury.      c) Basement Stuffing Department- The employer failed to ensure that all areas where hazards were created by pinch points, ingoing nip points and rotating parts on machines, such as on the Woburn #1 Setting Machine where two-hand tripping devices were not functioning , were provided with guarding to protect the operator from injury.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
