29 CFR 1910.147(c)(7)(i)(B): Affected employees were not instructed in the purpose and use of the energy control procedure.    On or about 2/7/2013, at the Flowery Branch facility, the employer did not provide LOTO Affected person training to those employees who are not providing service and maintenance to equipment in the facility but are working in and around areas where Authorized persons were providing that work.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.147(e)(3): Each lockout or tagout device was not removed from each energy isolating device by the employee who applied the device.    On or about 2/7/2013, in the production area, the employer retrained "master keys" to locks used for purposes of lockout/tagout in the event the locks needed to be removed by an employee other than the authorized employee that applied the lock.      In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.    On or about 2/7/2013, in the production area near the Annealing lines, the emergency eye wash was blocked by a catch basin tote and was not immediately available.

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items of this section.    On or about 2/7/2012, at the Flowery Branch location, the employer did not create specific written LOTO procedures for employees providing service and maintenance work to equipment at the facility. Deficiencies included: incomplete listing of energy sources, specific locations for control of energies and magnitude of energies involved on the equipment.     Koswire, Inc. located at 4600 Cantrell Road, Flowery Branch, Georgia was cited for a violation of this OSHA standard, 29 CFR 1910.147(c)(4)(ii) which was contained in OSHA inspection #315742239 (OIS#379182), citation 1, item 5. The citation became a final order on 9/13/2012.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points and rotating parts.    On or about 2/7/2013, at the Single Block lines 1B2401 and 1B2402, employees were exposed to amputation hazards as the employer did not ensure guarding for all ingoing pinch points and rotating parts. Available guarding did not fully enclose the hazards and/or limit employee entry into the hazard areas on the machines.     Koswire, Inc. located at 4600 Cantrell Road, Flowery Branch, Georgia was cited for a violation of this OSHA standard, 29 CFR 1910.212(a)(1) which was contained in OSHA inspection #315742239 (OIS#379182), citation 2, item 1. The citation became a final order on 9/13/2012.
