On or about March 05, 2015 machine operators were exposed to fire and explosion hazards during entry into the annealing line degreasing tanks where the employer developed, but did not implement a permit space program, in that employer did not complete entry permits, perform atmospheric monitoring, or train affected employees.  b.  On or about March 05, 2015 machine operators were exposed to fall hazards during entry into the first draw pits where the employer developed, but did not implement a permit space program, in that employer did not complete entry permits, or train affected employees.

On or about March 05, 2015 machine operators were exposed to electrical hazards  where there was outer jacket damage to the flexible cord supplying power to the first draw PM1 "Lamnea Bruk AB" mobile pointer machine; machine 8.0-1.0, number 42713-10.  d.  On or about March 05, 2015 machine operators were exposed to electrical hazards  where the flexible cord supplying power to the number seven portable welder/grinder station showed outer jacket damage.
