Section (5)(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of being struck by molten copper, steam and shrapnel.       On or about 2/2/2013, Outcast Area:  "Cooler" cylinders used for the drawing of copper wire were not provided with a means to release pressure such pressure relief valves.         Among other methods, a feasible abatement method to correct this hazard is to:            1) Select and include in construction only components that have a working rating equal to or greater than required to meet the maximum recommended conditions as referenced in Section 4.1.2 of ASTM E2349-12, Standard Practice for Safety Requirements in Metal Casting Operations, 2012.         2) Insure that any modifications or alterations to a piece of equipment or machinery which result in a change from the manufacturers original design and/ or intended method of operation shall be done under the supervision of a qualified engineer and shall comply with mandatory safety standards for that given category as referenced in Section 4.1.6 of ASTM E2349-12, Standard Practice for Safety Requirements in Metal Casting Operations, 2012.              3) Insure that any fluids (gas or liquid) are exhausted to locations that will not create a hazardous condition as referenced in Section 4.8 of ASTM E2349-12, Standard Practice for Safety Requirements in Metal Casting Operations, 2012.          4) Insure that if any modifications to equipment is performed, that the modifications have functional engineering drawings as is referenced in Section 5.1.2 of ASTM E2349-12, Standard Practice for Safety Requirements in Metal Casting Operations, 2012.               5) Reinstall, test  and maintain pressure relief valves that had been removed from the cooler cylinders.     Abatement documentation is required for this item.

Employees exposed to falls of over eight feet.       b) On or about 2/11/2013, Casting Platform:  An eighteen inch wide section of the platform was not provided with any fall protection.  Employees were exposed to falls of 7.75 feet.      c) On or about 2/11/2013, Casting Platform:  A three foot by six foot opening in the center of the elevated platform was not provided with any midrails or toeboards.  Employees exposed to falls of 7.75 feet.       d) On or about 2/11/2013,  South East section of the Casting Platform:  No midrails were provided for over twenty feet of platform edge.  A chain top rail had over ten inches of deflection. Employees exposed to falls of 7.75 feet.     e) On or about 2/11/2013, South West section of the Casting Platform:  A series of top rails and midrails had over twenty inches of deflection.  Employees exposed to falls of 7.75 feet.       f) On or about 2/11/2013, On top of the Caster/ Furnace:  A platform on top of the Caster/ Furnace was not provided with any fall protection.  Employees exposed to falls of fifteen feet.       g) On or about 2/11/2013, At the South corner of the Caster/ Furnace:  An access platform was not provided with fall protection.  Employees were exposed to falls of nine feet.       h) On or about 2/11/2013, At the South corner of the Caster/ Furnace:  A section of open sided plant floor at the slag removal area was not provided with fall protection.  Employees exposed to falls of approximately eight feet.     i) On or about 2/11/2013, Maintenance Shop:  A storage platform was not provided with fall protection.  Employees exposed to falls of nine and a half feet.     j) On or about 2/15/2013, In the Facility:  A Holland High Lift was not equipped with midrails in two locations.  Employees exposed to falls of over six feet.     k) On or about 2/15/2013, C13 Machine Power Platform:  The C13 power platform was not provided with a guardrail system.  Employees are exposed to falls of over seven feet.     l) On or about 2/15/2013, North East end of the Furnace Pit:  A section of guardrail was only supported by a center post, allowing for the sections on either side of the center post to pivot.  Employees were exposed to falls of eight feet.     m) On or about 2/15/2013, North East side of the Outokumph Furnace:  An eighteen inch section of guardrail system was missing.  Employees exposed to falls of nine feet.     n) On or about 2/15/2013, North East side of the Outokumph Furnace:  A top rail was at a height of thirty two inches and a mid rail at a height of eight inches above a small platform.
