29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or other group:  a) On or about 7/1/2016, Throughout the facility:  Operators were making die changes and working on machines after others had applied locks for them.  Operators were not in control of the keys to the locks.    Abatement documentation is required for this item.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or before 7/1/2016, Draw Machine Area:  The UDM 22 draw machine was operated with its guard not in place and with inadequate guarding.  Employees ex[posed to rotating parts and nip points.  b) On or before 7/1/2016, Draw Machine Area:  The Vaughn Dead Block draw machine was operated with its cover/ door open and interlock bypassed.  Employees are exposed to rotating parts.  c) On or before 7/1/2016, Draw Machine Area:  The Vaughn Bull Block draw machine was operated without proper guarding.  Employees exposed to rotating parts and nip points.  d) On or before 7/1/2016, Extruder Area:  A metal cutting alligator shear was operated without a properly adjusted guard and at times with no guard.  Employees exposed to shear points.    Abatement documentation is required for these items.   Tecnofil Chenango, SAC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 1086093, citation number 1, item number 3 and was affirmed as a final order on 3/7/2016, with respect to a workplace located at 40 South Main Street, Sherburne, NY.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  a) On or about 7/1/2016, Draw Bench Area:  Employees were exposed to the energized conductors in side of the electrical distribution box for the draw bench saw. .  Abatement documentation is required for these items.   Tecnofil Chenango, SAC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.303(g)(2)(i), which was contained in OSHA inspection number 1086093, citation number 1, item number 9 and was affirmed as a final order on 3/7/2016, with respect to a workplace located at 40 South Main Street, Sherburne, NY.
