29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the forks: a) At the site located at building 36 at Holly Heath Dr., a pallet used as a scaffold platform was not secured to the forks of a JCB Lift #184081. The exposed employee was not protected from falling by a standard guardrail system or personal protective equipment such as but not limited to a personal fall arrest system.. the employee wsa exposed to a 9 foot 4 inch fall.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) At the site located at building 36 Holly Heather Dr. an employee was exposed to a 10 foot 4 inch fall while decking a roof. The employee was working with no fall protection. First Construction Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 697438, citation number01, item number 001 and was affirmed as a final order on 12/22/12, with respect to a workplace located at 13162 Royal Pines Ave. Riverview, Fll. First Construction Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 316306877, citation number 01, item number 001 and was affirmed as a final order on 3/19/2012, with respect to a workplace located at Holmswood Greens, Lithia, Fl.
