29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:    (a) The employer did not establish a written respirator program for voluntary respirator wearers that covered required elements concerning medical evaluations and respirator cleaning, maintenance, and storage.    (b) A mechanic who wore a half-face cartridge style respirator on a voluntary basis while performing painting of boat bottoms had not been provided with a medical evaluation to determine his ability to safely use the respirator.    (c) A mechanic in the service department who wore a half-face cartridge style respirator on a voluntary basis while using cans of spray paint to paint small parts had not been provided with a medical evaluation to determine his ability to safely use the respirator.

29 CFR 1910.253(b)(2)(ii): Assigned storage spaces for cylinders was not located away from elevators, stairs, or gangways where cylinders could not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons:    (a) Approximately 3 compressed cylinders of acetylene and 3 compressed cylinders of oxygen were being stored upright and unsecured outside of the service department on an asphalt surface. The cylinders could be knocked over or damaged by passing equipment (e.g., forklift) or subject to tampering  by unauthorized persons.     (b) Four compressed cylinders of oxygen were stored outside of the service department on a bottle rack adjacent to the cylinders referenced in instance (a) above. The cylinders were not secured from falling with the available chain that was attached to the rack.
