29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: A crew of three employees was not protected against falls from a roof approximately 15 feet to 19 feet above ground level while installing plywood decking. The crew did not have protection such as, but not limited to, guardrails, personal fall arrest systems, or safety nets.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder. At this construction project, employees installing plywood decking on a roof used a portable ladder to access the upper roof work area which did not extend 3 feet above the roof line. The ladder was not secured to prevent movement.
