Section 5(a)(1) of the Occupational Safety and Health Act: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the potential hazard of exposure to high concentrations of anhydrous ammonia: a.) On or about February 11, 2013, and at times prior thereto, the discharge from the two pressure safety relief valves of the Ammonia High Pressure Receiver (HPR) was not piped to a safe location. The discharge from the valves was immediately above the HPR. b.) On or about February 11, 2013, and at times prior thereto, the door from the Anhydrous Ammonia Machinery Room to the manufacturing facility, was not an approved self-closing, tight-fitting fire door which opened outward. The door was not equipped with panic type hardware. Note 1: Abatement certification and documentation ARE required for this item
