This exposed employees to delays in existing the vessel causing smoke inhalation and fire hazards.         (b) F/V Blue Gadus, Bait Room: On or about 16 February 2013 and times prior thereto; the emergency exit plunger for the bait room was not functional; exposing employees to hypothermia hazards.

29 CFR 1918.11(a):     The employer used the vessel cargo handling gear without ascertaining that the vessel had a current and valid cargo gear register and certificates in accordance with ILO 152 recommendations.      Blue Gadus forward deck area:  On or about 16 February 2013 and times prior thereto, the Center Bow Cargo Boom did not have an annual marine crane certification accomplished.  The last time the crane was inspected was in June 2008.  This condition exposed employees to a struck by hazard.
