29 CFR 1918.11(a): The employer used the vessel's cargo handling gear without ascertaining that the vessel had a current and valid cargo gear register and certificates in accordance with ILO 152 recommendations. F/V Blue Gadus, City Dock, forward deck area: On or about 5 February 2015 and times prior thereto, the Center Bow Cargo Boom did not have an annual marine crane certification accomplished. The last time the crane was inspected was in September 2013. This condition exposed employees to a struck by hazard. The F/V Blue Gadus was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1918.11(a), which was contained in OSHA inspection number 891205, citation number 1, item number 2 and was affirmed as a final order on 15 May 2013, with respect to a workplace located at City Dock, Dutch Harbor, AK.
