This condition exposed employees to an electrical shock and burn hazard. (B) Maintenance shop: On or about 14 February 2013 and times prior thereto, a black extension cord exhibited strain relief and electrical wires were exposed. This condition exposed employees to an electrical shock and burn hazard.
