29 CFR 1910.212(a)(5):   Fan blade guard(s) were not provided where the periphery of the blades was less than seven feet from the floor or working level.  Maintenance Shop:  On or about 14 February 2013 and times prior thereto, the 120 volt fan in the men's restroom was missing the fan blade cover.  This condition exposed employees to a caught in and pinching hazard.

29 CFR 1910.305(g)(2)(iii):   Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws.  (a) Maintenance shop:  On or about 14 February 2013 and times prior thereto, two yellow ceiling mounted electrical extension cord rollers exhibited strain relief and electrical wires were exposed.  This condition exposed employees to an electrical shock and burn hazard.  (B) Maintenance shop:  On or about 14 February 2013 and times prior thereto, a black extension cord exhibited strain relief and electrical wires were exposed.  This condition exposed employees to an electrical shock and burn hazard.

29 CFR 1917.42(b)(2)(ii):   The employer used wire rope or wire rope slings that had kinking, crushing, bird caging, or other damage which distorted the wire rope structure.  (a) Terminal one, Gear van:  On or about 14 February 2013 and times prior thereto, 20 foot, West wire rope, 7/8 diameter was bird caged.  This condition exposed employees to a struck by hazard.  (b) Terminal one, Gear van:  On or about 14 February 2013 and times prior thereto, 15 foot Pentco wire rope, 7/8 diameter was bird caged.  This condition exposed employees to a struck by hazard.

29 CFR 1917.151(g)(4):   Work rests on fixed grinding machines were not adjusted closely to the wheel with a maximum opening of 1/8-inch (3.18 mm) and securely clamped.  Terminal One, Machine shop:  On or about 14 February 2013 and times prior thereto, the work rest on the Milwaukee Bench Grinder, Serial number W9-95, measured approximately 3/8 inch from the abrasive wheel. This condition exposed employees to a struck by hazard.

29 CFR 1917.152(e)(2)(ii):   Arc welding or cutting cable was used when it was not free from repair or splice for 10 feet (3 m) from the electrode holder.  Terminal One, Machine Shop:  On or about 14 February 2013 and times prior thereto, the Thermal Dynamics Pak Master 100 Plasma cutter had a cut approximately 60"inches from the stinger and was taped over with electrical tape.  This condition exposed employees to an electrical shock hazard.
