29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employees from the hazards identified in the hazard assessment: a) Packers Sanitation Services Inc. at JBS USA, LLC, 800 North 8th Avenue, Greeley, CO 80631: On or about December 4, 2012, the employer provided a shock absorbing lanyard for employee use when cleaning above the down puller machine while it was running, and the 6 foot lanyard extension did not protect employees from a fall from the landing of 54 inches. Condition exposed employee to amputation and crushing hazards. Abatement Note: A fall restraint/positioning lanyard, without a shock absorber, was needed to prevent the employee from falling into the downpuller machine.

29 CFR 1910.29(a)(4)(ii): A manually propelled mobile ladder stand was provided without a positive wheel or swivel lock to prevent movement: a) Packers Sanitation Services Inc. at JBS USA, LLC, 800 North 8th Avenue, Greeley, CO 80631: On or about December 4, 2012, the employer allowed employees to use a manually propelled stainless steel mobile ladder stand for use in cleaning the upper conveyors of the fabrication area, and this ladder stand did not have a positive wheel or swivel lock to prevent movement. Condition exposed cleaning contractor employees to fall hazards of up to 6'.

29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading Exit: a) Packers Sanitation Services Inc. at JBS USA, LLC, 800 North 8th Avenue, Greeley, CO 80631: On or about December 4, 2012, the employer did not ensure that the room adjacent to the fabrication area, where the cleaning employees hooked up their water hoses, included an exit sign. This condition would cause employees to not be able to identify the door as a means of egress from the building in the event of an emergency situation.
