29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4): On or about February 19, 2013, in the shop production area: a) Two employees performing millwork were exposed to amputation hazards while changing saw blades on the Grizzly 12" Table saw when the employer did not have a program developed and implemented that informed the employees how to recognize and minimize lockout tagout/hazards. b) One employee performing millwork was exposed to amputation hazards while changing blades on the Edge bander when the employer did not have a program developed and implemented that informed the employees how to recognize and minimize lockout/tagout hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about February 19, 2013, in the shop production area: a) Employees performing millwork were exposed to an electric shock hazard when they used the 115 volt, 17 amp, Kreg machine that had strain relief damage on the pedal cord leading into the machine and the end leading to the pedal. b) Employees performing millwork were exposed to an electric shock hazard when they used the Grizzly dust collector connected to the edge bander that had strain relief damage to the end of the power cord leading into the control box and the end of the cord spliced into a metal junction box.
