A noise monitoring program was not developed and implemented. The exposure was derived from a sample taken over 454 minutes period, on or about 2/27/13.    b) H & D Pallets, LLC, Lula, GA - An employee cutting lumber using a miter saw was exposed to noise levels at a TWA of 90.5 dBA, which exceeds the noise action level of 85 dBA. A noise monitoring program was not developed and implemented. The exposure was derived from a sample taken over 498 minutes period, on or about 2/27/13.    c) H & D Pallets, LLC, Lula, GA - An employee building pallets using a nail gun and hammer was exposed to noise levels at a TWA of 95.9 dBA, which exceeds the noise action level of 85 dBA. A noise monitoring program was not developed and implemented. The exposure was derived from a sample taken over 427 minutes period, on or about 2/27/13.    d) H & D Pallets, LLC, Lula, GA - An employee building pallets using a nail gun and hammer was exposed to noise levels at a TWA of 94.5 dBA, which exceeds the noise action level of 85 dBA. A noise monitoring program was not developed and implemented. The exposure was derived from a sample taken over 456 minutes period, on or about 2/27/13.
