29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  On or about 8/8/2017 at 464 Garrison Shoals Road, Lula, GA, employees expected to fight incipient level fires were exposed to smoke inhalation and burn hazards where the employer failed to insure portable fire extinguishers were not blocked. A portable fire extinguisher was identified as blocked by a pallet for the past week.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  On or about 8/8/2017 throughout the facility the employer failed to provide the appendix "D" to employees that were provided tight fitting paper dust masks for wear.  H & D Pallets, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.134 (c)(2)(i), which was contained in OSHA inspection number 891863, citation number 02, item number 01 and was affirmed as a final order on 3/26/2013, with respect to a workplace located at 464 Garrison Shoals Road Lula, GA.

A portable fire extinguisher was identified as expelled since the fire in October, 2016.  H & D Pallets, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.157(e)(2), which was contained in OSHA inspection number 891345, citation number 01, item number 1a and was affirmed as a final order on 3/26/2013, with respect to a workplace located at 464 Garrison Shoals Road Lula, GA.

29 CFR 1910.157(e)(3): Portable fire extinguishers were not subjected to an annual maintenance check:  On or about 8/8/2017 at 464 Garrison Shoals Road, Lula, GA, employees expected to fight incipient level fires were exposed to smoke inhalation and burn hazards where the employer failed to inspect the portable fire extinguishers on an annual basis. Portable fire extinguishers were last inspected October 20, 2015. Additionally, a portable fire extinguisher was identified as expelled since the fire in October, 2016.  H & D Pallets, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.157(e)(3), which was contained in OSHA inspection number 891345, citation number 01, item number 1b and was affirmed as a final order on 3/26/2013, with respect to a workplace located at 464 Garrison Shoals Road Lula, GA.
