29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas:    On or about February 26, 2013, at the above addressed worksite, employees working on the front wall of the building were exposed to tripping hazards from debris and lumber with nails located adjacent to the entrance and along the front of the building. Crossbraces were used as a means of access:    On or about 02/27/2013, at the above addressed worksite, employees were exposed to a fall hazard while working on a scaffold that did not have a ladder for access.
