29 CFR 1910.147(d)(3):  Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):       (a)  Employees changed the blade on the DoAll Vertical Band Saw without first isolating the energy source.        (b)  Employees cleaned and replaced the rollers and knives on the #11, #16 and #18 coaters, without first isolating the energy sources.        (c)  Employees cleaned and replaced the rollers and knives on the #475 Coater, without first isolating the energy sources.          In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.147(d)(4)(i):  Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees:       (a)  Employees changed the blade on the DoAll Vertical Band Saw without first affixing a lock to each energy isolating device.        (b)  Employees cleaned and replaced the rollers and knives on the #11, #16 and #18 coaters, without first affixing a lock to each energy isolating device.        (c)  Employees cleaned and replaced the rollers and knives on the #475 Coater, without first affixing a lock to each energy isolating device.        Troy Laminating and Coating, Inc. was previously cited for a violation of this occupational safety and health standard, or its equivalent standard, which was contained in OSHA inspection number 314343096, Citation Number 1, Item Number 4 and was affirmed as a final order on August 11, 2010, with respect to a workplace located at 421 S. Union Street, Troy, OH 45373.          In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.147(c)(6)(i)(C):  Where LOTO was used for energy control, the periodic inspection did not include a review between the inspector and each authorized employee, of that employees responsibilities under the energy control procedures being inspected:      (a)  There were no periodic reviews between an inspector and the authorized employees, to include a demonstration by the authorized employees of specific energy isolation procedures.      Troy Laminating and Coating, Inc. was previously cited for a violation of this occupational safety and health standard, or its equivalent standard, which was contained in OSHA inspection number 314343096, Citation Number 1, Item Number 2 and was affirmed as a final order on August 11, 2010, with respect to a workplace located at 421 S. Union Street, Troy, OH 45373. 29 CFR 1910.147(c)(7)(i):  The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:       (a)  Not all employees working in the maintenance shop were provided with training on the energy control program, such as when and how to attach a lock to the energy isolating device, prior to changing the blade on the DoAll Vertical Band.      (b)  Employees operating, or performing maintenance on, the #11,  #16 and #18 coaters, were not adequately trained on the energy control program, such as when and how to attach locks to the energy isolating devices, prior to cleaning and replacing rollers and knives.      (c)  Employees operating, or performing maintenance on, the #475 Coater, were not adequately trained on the energy control program, such as when and how to attach locks to the energy isolating devices, prior to cleaning and replacing rollers and knives.
