29 CFR 1910.119(d)(3)(i)(B): The employer did not include all equipment that was part of the process on piping and instrument diagrams: Employees were exposed to hazards associated with release of ammonia in that the employer had not verified that all P&IDs were up-to-date and accurate for use by Hiland Dairy Foods employees and contractors for the equipment listed below: a) High Stage Compressor #1 did not have the high temperature cutout instrument listed on PID6. b) High Stage Compressor #2 did not have the high temperature cutout instrument listed on PID8. c) Oil Drain valves show a cap on the P&IDs instead of the quick disconnects that are actually installed.

29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices: Employees were exposed to hazards associated with release of ammonia in that the employer did not ensure that the linear liquid indicators were fitted with check-type shutoff valves as required in IIAR 2-2008, Section 12.1.2.
