29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about March 6, 2012, at 3813 Summit View Lane, Snellville, Georgia:    An employee loading asphalt shingles onto an elevator was exposed to struck-by hazards of falling asphalt shingles from the employees unloading asphalt shingles from the elevator above him when he did not wear a hard hat.

29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about March 6, 2013, at 3813 Summit View Lane, Snellville, Georgia:    (a)  Two employees performing roofing work on a house were exposed to a fall hazard that ranged from approximately 8.8-feet to 13.6-feet when they worked at the unprotected edge without fall protection.     (b)  Two employees loading shingles on the roof of a house were exposed to fall hazards of approximately 8.8 to 13.6-feet when they did not use fall protection.
