29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used.    a) Front of facility located at 3999 Austell Road, Austell, Georgia.  The employer did not provide a safe means of accessing a mobile fabricated frame scaffold working platform exposing an employee to fall hazards of approximately 13-feet 5-inches.

29 CFR 1926.451(g)(1): 29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level by fall protection established in accordance with paragraphs (g)(1)(i) through (vii) of this section:   a) Front of facility located at 3999 Austell road, Austell, Georgia. An employee was not protected from falling approximately 13-feet 5-inches from a mobile fabricated frame scaffold working platform to a concrete sidewalk below by the use of a guardrail system, safety net system, or personal fall arrest systems.

29 CFR 1926.452(w)(1): Scaffolds were not properly braced by cross, horizontal, or diagonal braces, or a combination thereof, to prevent racking or collapse of the scaffold and/or to secure vertical members together laterally so as to automatically square:   a) Front of facility located at 3999 Austell Road, Austell, Georgia. An employee was exposed to fall hazards of approximately 13-feet 5-inches as the frames of a mobile fabricated frame scaffold system were not properly braced. Only the outer sides of the fabricated frame scaffold system were diagonally braced to prevent collapse.

The employer did not develop, implement, or maintain a written hazard communication program which at least describes forms of warning, safety data sheets, and employee information and training as it pertains to, in this case, an unknown solution used in removing excess mortar and the cleaning of bricks.
