29 CFR 1926.502(a)(2): The employer did not provide and install all fall protection systems required by this subpart for employee(s) and did not comply with all other pertinent requirements of this subpart before employee(s) began the work that necessitates the fall protection.   a) New residential construction located on Lot 26 at 300 Blackberry Court, Pendergrass, GA 30567, Materials hoist: On or about 5/14/2026, the employer exposed employees to fall hazards of approximately 10-feet to the lower ground level, in that the employer allowed for knots to tied into the wearers back in place of the rope grab use.

29 CFR 1926.552(a)(4): Hoisting ropes were not installed in accordance with the wire rope manufacturers' recommendations:  a) New residential construction located on lot 26 at 300 Blackberry Court, Pendergrass, GA 30567, Materials hoist: On or about 5/14/2026, the employer exposed employees to struck by hazards while operating the TS 250 materials hoist, in that the manufacturer's required wire rope was not installed.
