(a) (a)	On or about March 8, 2013 an employee was potentially exposed to serious injury from a fall to the water and/or metal structure when accessing the Tidewater Barge 24 from the Boardman Container Terminal gantry crane at 79827 Ullman Lane in Boardman, Oregon.  The employee stepped directly from the gantry crane rail to the barge instead of using a safe means of access that was provided and available (i.e., metal platform extensions).
