29 CFR 1917.45(f)(4): Ladders, stairways, stanchions, grab irons, foot steps, or equivalent means were not used on cranes or derricks to ensure safe access to walkways, cab platforms, crane cab and/or any portion of the superstructure that employees were required to reach. (a) (a) On or about March 8, 2013 an employee was potentially exposed to serious injury from a fall to the water and/or metal structure when accessing the Tidewater Barge 24 from the Boardman Container Terminal gantry crane at 79827 Ullman Lane in Boardman, Oregon. The employee stepped directly from the gantry crane rail to the barge instead of using a safe means of access that was provided and available (i.e., metal platform extensions).

29 CFR 1917.152(h)(1): Employees were not protected from radiant energy eye hazards by spectacles, cup goggles, helmets, hand shields or face shields with filter lenses complying with the requirements of this paragraph. (a) On or about March 8, 2013 the inspection revealed that employees performing welding work were not wearing face shields with the minimum protective shade rating for the task. Located at the Boardman Container Terminal at 79827 Ullman Lane in Boardman, Oregon.
