29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts:    On or about March 13, 2013, and times prior to, at the facility located at 3300 South Woodrow in Little Rock, Arkansas;    (a)    In the Bandsaw Assembly area, approximately 18" of the unused portion of the cutting blade on the Hyd-Mech S-20P Series III bandsaw was not guarded.  This condition exposed employees to contact with the cutting blade.    (b)    In the Conveyor Assembly area, approximately 6" of the unused portion of the cutting blade on the Delta Milwaukee bandsaw was not guarded.  This condition exposed employees to contact with the cutting blade.    (c)    In the Machine Shop area, a guard was not furnished over the rotating chuck on the Jet lathe.  This condition exposed employees to the hazard of contact with rotating parts.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  On or about March 13, 2013, and times prior to, at the facility located at 3300 South Woodrow in Little Rock, Arkansas;  (a)    In the Machine Shop area, a guard was not furnished on the Bridgeport Miller, J134376 and Lagun Republic Miller, FTV-1.  This condition exposed employees to contact with the point of operation.  (b)    In the Machine Shop area, the rotating drill bit on the Rockwell drill press was not guarded.  This condition exposed employees to punctures and contact with the rotating bit.  (c)    In the Conveyor Assembly area, the rotating drill bit on the Atlas drill press was not guarded.  This condition exposed employees to punctures and contact with the rotating bit.

(b)    In the Conveyor Assembly area, the belt and pulley to the motor drive for the Atlas drill press, were not guarded.    These conditions exposed employees to the hazard of contact with ingoing nip points.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  On or about March 13, 2013, and times prior to, at the facility located at 3300 South Woodrow in Little Rock, Arkansas, in the Conveyor Assembly area, the 110 volt metal framed receptacle that supplied power to a bandsaw and radio was suspended above the conveyor table.  The receptacle was listed to be installed within a wall as a permanent fixture.
