29 CFR 1910.23(a)(9): Each floor hole, into which persons could not accidentally walk, was not protected by a cover that leaves no openings more than 1 inch wide.     On or about September 7, 2016, and times prior thereto, in the shop, where employees frequently walk there is a greater than an inch uncovered floor hole, which exposes them to trip and fall hazards.

29 CFR 1910.37(a)(3):  Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.     On or about September 7, 2016, at the production area, the employer did not ensure the exit route was free and unobstructed. This condition created delay in exiting the building in an emergency, and  trip and fall hazards in a fire.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  On or about September 7, and times prior thereto, in the production area, employee(s) operated a Cincinnati Hydraulic Press Brake in which its foot pedal cord outer  jacket was torn, exposing the cord insulation underneath. This condition exposed employees to an electrical shock hazard.
