29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function: On or about 3/12/2013, at the above addressed jobsite, employees performing safety monitor duties were engaged in other activities taking their attention from the monitoring functions. Instance 1: At Bldg. #1, an employee engaged in safety monitor duties was not able to observe the employee working on the roof due to his back being turned away from the employee on the roof as he was transferring building material (Firestop Drywall) to another location on the top level, exposing employees to a 9ft & 10½ inches fall hazard. Instance 2: At Bldg. #2, an employee engaged in safety monitor duties was not able to observe the employee working on the roof due to his back being turned while engaged in his phone call, exposing employees to a 9ft & 10½ inches fall hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about 3/12/2013, at the above addressed jobsite, employees engaged in residential construction activities were not protected from fall hazards: Instance 1: An employee at Bldg #9 was not protected from fall hazards while cutting plywood on the 2nd level approximately 10' from ground level. Instance 2: Two employees at Bldg #6 were not protected from fall hazards while installing floor trusses on the 2nd level approximately 9' from ground level. Instance 3: Two employees at Bldg. #4 were not protected from fall hazards while installing plywood onto the trusses of a residential home.
