29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: The employer does not ensure that each employee on a walking/working surface with an unprotected side or edge which is 6 feet or more above a lower level is protected from falling. This violation occurred on or about March 14, 2013, on the north side of the jobsite where an employee was exposed to a fall hazard of approximately 10 feet, 6 inches when decking the roof of a building without using a fall protection system. Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each employee on a walking/working surface with an unprotected side or edge which is 6 feet or more above a lower level is protected from falling.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: The employer does not ensure that a training program is provided for each employee who might be exposed to a fall hazard. This violation occurred on or about March 14, 2013, at the jobsite where an employee was exposed to a fall hazard when decking the roof of a building under construction without fall protection training. Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that a training program is provided for each employee who might be exposed to a fall hazard.
