29 CFR 1926.303(b)(1): Grinding machines were not equipped with safety guards in conformance with the requirements of ANSI B7.1-1970, Safety Code for the Use, Care, and Protection of abrasive wheels, and paragraph (d) of this section: a) At the construction site, in section A2, apartment 1451, on or about March 14, 2013 - Employees were exposed to struck-by hazards when operating a Dewalt 4-1/2 inch, 11,000 rpm Angle Grinder (Model Number D28110, Serial Number 13_262) which was not guarded.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who was on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: (a) At the construction site located at 3788 Richmond Avenue, in courtyard 3, on or about March 14th, 2013 - An employee was exposed to fall hazards in excess of 30' while installing railing on apartment balcony areas without fall protection.
