29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:  a. On or about 03/14/2013, at the job site - an employee installing Tapcon screws from a ladder was exposed to a 6 feet fall hazard, in that, he was permitted to stand on the top step of an A-frame ladder. The ladder was not fully extended apart and one spreader or locking device was not in the lock position.

29 CFR 1926.1060(a)(1)(i): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the nature of fall hazards in the work area:   a. On or about 03/14/2013, at the job site - an employee installing Tapcon screws from a ladder was exposed to a 6 feet fall hazard, in that, he was not trained in the use ladders.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On or about 03/14/2013, at the job site - employees working inside the units were exposed to a 9 feet 4 inches fall hazard (from eave to ground), in that, guard rails were not installed in the wall openings for windows.    b. On or about 03/14/2013, at the job site - an employee working inside the units was exposed to 9 feet 4 inches fall hazard, in that, he was permitted to use the window opening to access the roof to move from one apartment unit to another.  FIRST CONSTRUCTION SERVICES INC. WAS PREVIOUSLY CITED FOR VIOLATIONS OF THE OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(B)(13) WHICH WAS CONTAINED IN OSHA INSPECTION 315484444, CITATION 02 ITEM NUMBER 01, ISSUED ON 04/19/2012, AND BECAME A FINAL ORDER ON 07/25/2012, WITH RESPECT TO A WORKPLACE LOCATED ON 11395 BAYMEADOWS ROAD EAST JACKSONVILLE, FL 32256. AND OSHA INSPECTION 316306877, CITATION 01 ITEM NUMBER 01, ISSUED ON 02/21/2012, AND BECAME A FINAL ORDER ON 03/19/2012, WITH RESPECT TO A WORKPLACE LOCATED ON HOLMSWOOD GREEN LITHIA, FL 32569.
