On or about 03/14/2013, at the job site - an employee installing Tapcon screws from a ladder was exposed to a 6 feet fall hazard, in that, he was permitted to stand on the top step of an A-frame ladder. The ladder was not fully extended apart and one spreader or locking device was not in the lock position.

On or about 03/14/2013, at the job site - employees working inside the units were exposed to a 9 feet 4 inches fall hazard (from eave to ground), in that, guard rails were not installed in the wall openings for windows. b. On or about 03/14/2013, at the job site - an employee working inside the units was exposed to 9 feet 4 inches fall hazard, in that, he was permitted to use the window opening to access the roof to move from one apartment unit to another.
